What does it mean when your ALT is high?

Today we’ll be talking about ALT.

And in particular, what a high result means for you.

ALT (which stands for alanine aminotransferase), is an enzyme used to measure risks to the liver.

By running a blood test for it we can:

  • Check for liver diseases. Cirrhosis and hepatitis in particular.
  • Check for liver damage,?but not to what extent
  • In the case of jaundice, determine whether its cause was from disease or blood disorder.
  • Assess whether treatment for liver disease should take place
  • Analyze the effects of potentially liver harming medicines
  • Analyze if progress is being made in treatment for liver disease
?In the post we’ll cover:
  • What exactly constitutes a high test level.
  • What’s potentially causing the high blood levels for you.
  • How to determine the severity of your results.
  • What you can do next to help improve your condition.

What is considered?a high ALT level?

Basically, any reading above 55 units per liter (U/L).

When this shows up in your alt results there is probably an unhealthy amount of the enzyme present in your bloodstream.

Under normal conditions, these enzymes stay in liver cells. But when there’s damage in the liver the enzymes spill out into the bloodstream and circulate through the body.

Low Below 7 U/L
Normal Between 7 – 55 U/L
High Above 55 U/L

Mildly-elevated levels range from two to three times the normal to high mark. If your levels are around 55 to 200 U/L it’s not a time to panic, but you should look into it further.

Very high levels in the blood can go as high as multiple thousands?of units per liter. As high as 2,000-20,000 U/L!

What could be causing your high alt levels?

There’s a broad range of potential causes for elevated ALT levels.

Some are more severe than others, with some results coming from acute liver damage and others from chronic liver conditions.

In any instance, all cases of the enzyme becoming an elevated point of?damage that has taken place. Otherwise, it wouldn’t be in the blood in high amounts.

But you should determine whether it’s a dangerous or more common, less threatening reason.

Moderate risk ALT Symptoms

This is the 55 to a few hundred U/L level and is not too serious (in most cases).

There aren’t many noticeable symptoms associated with slightly elevated ALT levels. When slightly elevated levels are present it can be an indicator that more risk could be on the horizon, though.

So if your numbers are ranging from 55 to a few hundred U/L you’re advised to read on and assess whether you could be at risk for more pronounced issues in future.

Because remember, once the markers get into the thousands we’re looking at something more serious.

Moderate to high-level ALT symptoms can include:

  • General tiredness
  • mild fever
  • lack of appetite
  • pain in the abdomen
  • itching
  • nausea/vomiting
  • mild jaundice (when your skin gets yellow)

Possible causes for these moderately upped numbers may include:

  • Reactions to medications
  • Physical injury
  • poor diet/lots of processed foods
  • mild inflammation
  • alcohol bingeing
  • prolonged Tylenol/acetaminophen use
  • antibiotic use
  • pregnancy

Higher risk ALT Levels (>1,000 U/L)

If your levels are really high, there’s a lot of potential causes. Though some are very interrelated.

Below is a list of what might be ailing you:

  • hepatitis of the liver (either acute – A or B – or chronic – C)
  • cirrhosis of the liver
  • liver tissue death (also called necrosis)
  • Damage from alcohol abuse also called alcoholic fatty liver
  • Iron buildup in the liver (genetic)
  • Diminished blood flow to the liver
  • liver tumors/cancer
  • Severe drug overdose (often Tylenol, sometimes antibiotics, pain meds, psychiatric meds, seizure meds, wild mushroom poisoning etc…
  • Wilson’s disease (copper buildup in the liver)
  • autoimmune diseases
  • IBD (Crohn’s & ulcerative colitis)
  • Obesity

How to determine the severity of your ALT results

Whether you’re sick and likely to have a chronic disease or are a?healthy individual – if you have moderate to high aminotransferase results it’s important to investigate further what might be going wrong.?A thorough medical examination should?be conducted.

This would include letting your doctor know about any over-the-counter drugs or herbs you’ve taken recently.

For example, sometimes Chinese herbs that are widely prescribed can?cause cirrhosis in certain people.

Additionally, history of blood transfusion, drug use, sexual history (related to sexually derived hepatitis), alcohol consumption and potential foodborne illnesses should be investigated.

In addition to collecting that personal?data, more tests to screen the liver should be conduction.

  1. Thanks for explaining a little more about the mildly elevated & highly elevated levels.

    Trying to find info on what those actual levels are (eg. 55-200, >1000 etc.) is virtually impossible.

    1. If AST to ALT ratio is equal to one, there are chances of heart failure, ischemic heart diseases and hepatitis.
      If AST to Alt ratio is > 2.5, it indicates alcoholic fatty liver and cirrhosis.
      If AST to ALT ration is ALT. But, if the disease progresses or cirrhosis develops, the AST > ALT.

      I’d definitely have an ultrasound

      1. ultrasound can only detect scarring and tumors in pretty sure. need more bloodwork to find problem, if it’s something that could cause liver scarring or cancer(hep) then they will def order an ultra/fibrosis/ biopsy,etc

    My AST level came back (53) and ALT( 71) My doctor called with the results and gave me the 3rd degree as to my drinking habits. Are these numbers dangerously high?

    1. This could be from alcohol and if you drink then yes this would be why. If not, It is mildly elevated and should be monitored. It could be from any of the things listed below.

    2. Hi they can easily spiral higher so just be aware and keep on top of it. I’m 41 year old female and told today mine is 132 and my cholesterol is 7. So I’m changing my diet and I’ve not had any alcohol for 15days and I’ve got to quit alcohol altogether but good news is it’s not too far that I can’t fix it. Scared wasn’t the word!         1. That’s exactly what I was going to say! I have had my levels be low and then high and then back to normal. Blood work only shows a screenshot of what is going on at that moment which could be anything from mild things like antibiotics vs something more serious. Best thing to do is just keep an eye on it and don’t worry until you have a reason.

    1. I would say you should take a liver function test like the one listed in the article and monitor your results, eat healthy, exercise. in case, doctor told you you have a health or liver problem, don’t panic, there should be medication for most problems.

  3. I have been diagnosed with Acute hep b, my ALT level was 1950 and today it’s gone down to 1500 apparently it’s not unusual to go that high and not have liver damage

  8. My ast is/was 71 alt 52. When I had my blood drawn I was getting a good amount of exercise in for a boxing tournament. Roughly 8 hours 4 days a week. That wasn’t enjoyable. Next year hopefully my levels will decrease.

  9. I have had elevated Liver counts for a long long time (3-4 years) there are still no answers as to what could be causing it. All Hep tests came back negative, no HIV, no drinking history, no drug abuse, It all started when I had my first son 5 years ago. Anyways just had my second son 10 months ago. Got my blood back and its still elevated by A LOT. ALK Phos 443, ALT 166 AST 88. Any ideas out there?

  12. My Alt is nearly 4,000 and my Ast nearly 2,000. I am positive for Hep c. With a viral load of 21 something. I don’t know what this means as my Dr refused to see me telling the nurse there was nothing more she could do for me. I am scheduled for an ultrasound today and a gastroenterologist the end of the moth but quite frankly I’m scared I’m dying. All I was to do is sleep. I’ve no energy. Tired all the time and so itchy! My stool is very light and I can’t eat. I feel so full all the time. Can anyone help???

    1. alt and ast are liver enzymes releases by liver when under “attack” by your hep c. elevelated levels do not necessarily mean damage, only fibrosis,ultrasound,biopsy,etc. can determine this. These enzymes are more markers for something being wrong. Since you know you are hep c pos, you know why yours are high. As I mentioned, this does not mean for sure there is any significant damage (scarring) to your liver. Fibrosis test can be good indicator and it’s non-invasive (unlike biopsy)
      Hep c can make ppl nauseous, and also desribe the symptoms you mentioned .
      You need treatment. First they have to see how much damage there is. If you aren’t dying basically, they will go over treatment options with you. I completed Mavyret for 8 weeks and hep was undetectable at week 3, however my alt was still at 62. my ast was normal (33) and viral load 0 from almost 500,000!
      I suggest if you cannot afford it, and it’s possible get on medicaid (welfare insurance) it will pay for your treatment. it’s worth being broke for a few months, especially when dying of liver failure or cancer is possible. I have seen people die of hep c, and trust me it is horrible. your blood fills with bile and ammonia which get into your brain. it looks like you are pregnant because liver becomes so dissented . get the ultrasound, and do whatever you must do to get treatment. if you are using drugs the insurance won’t approve bc there is too high risk of reinfection or non-compliance. I couldn’t afford insurance, so I quit my job and 90 days later got on welfare. I was approved for $ 30,000 treatment and was “cured”. i wish this for all people with this disease, bc its hard to move on with life when knowing you have a disease that makes you sick and may kill you. I show my gratitude by living healthy and obstaining from drugs and alcohol…and trying to help others with this disease. I was lucky I was able not to work, not all people have this choice, but there are studies, there are ways. be vigilant. be clean and sober. show the doctors you are willing to do this and they will keep applying to insurance co if rejected first time (I was the 1st time). Until you can get treated you can take milk thistle, eat healthy, take a daily vitamin w/o iron, drink lots of water, light exercise, and avoid red meat, and fatty foods. This can do wonders and even lower enzyme levels and sometimes even viral loads. And NO alcohol. for hep c,it’s like throwing gas on a fire. Some people have it for years (I had it for 20) and aren’t sick, and one day wake up yellow and sick and are told they need a transplant. many don’t get them, and die a terrible death. I hope this makes you take this serious. We o my have 1 life and 1 body. we can’t fix mistakes of the past, but we can keep from repeating them. Take my advice and one day you will see the words “not detectable” next to HVC RAN. and it will be a good day. stay strong and don’t give up. the squeaky wheel gets oiled.

  13. I am 53 and have Hashimoto’s auto immune thyroid desease, suspected reactive EBV and CFS. For the first time my AST was 59 and my ALT was 108. I have high cholesterol even though I eat very well. I do not drink. And it seems my thyroid function is hypo at the moment. It fluctuates. And we tweak the armour I take accordingly. My doc did put me on lipitor for a month and then took me off and redid my labs a month after. My levels are still elevated but only slightly. I realize that thyroid and auto immune function can cause liver labs to elevate. But he was so shocked that my liver seems extraordinarily sensitive to a cholesterol drug. I fear that since my immune system attacked my thyroid its now going after my liver. I have been running daily low to mid grade fevers for almost two years now.

    1. interesting. seems to be a correlation between high hdl/lipid profiles and elevated enzymes/liver damage. I have hward lipitor can raise levels, as many medications (among other things can) . My only suggestion besides a basic metabolic panel, biliburin direct, is to cut out trans fats, and up fiber and protien and water intake. Also milk thistle has shown some promise of helping with cholesterol and high enzyme levels. I know someone who actually cleared hep c with actual thistle root. they took it for a year I think, and they still have undetectable levels. it’s not an fda med, soon would ask doc about any possible interactions, but risk vs. possible reward looks very promising for this supplement.

    1. alt is slightly elevated.ast within range. this could be anything. I suggest a comprehensive metabolic panel . it will give you a beyter idea. also a hcv rna test. if they all look good i wouldn’t worry. could be medicine, exercise, anything or nothing. do you drink? are you on medication? work hard labor? take meds/drugs? if anything like that it could give slightly elevated enzymes. clean up diet and try fasting before blood work and get tested again. don’t exercise a cpl days before. of still elevated get the tests I suggested. there are places online that will do them, this will give you a better idea if there is a problem. honestly, your levels aren’t alarming so I wouldn’t be too concerned, but better safe than sorry.

  14. I was just told today that my liver levels are around 71-73… over the last 2 years was told there were trace amounts of blood in my urine, but that it turned out to be fine. Over the last 3 years I’ve been diagnosed with acne medication induced lupus, hashimoto’s, carpal tunnel, fibromyalgia, lactose intolerance, and low Vitamin D (all but the lupus has stuck as diagnosis). I get my blood work done every 3 months and I swear every time they stick me they FIND SOMETHING ELSE!!! Now it’s my friggen liver. I’m ready for all this shit to be done.

    I tried 24 times in 12 years to kill myself (primarily using medications to overdose with, from 1994-2006, each time being told I shouldn’t still be alive). When I was 17 I was told (after spending a week unconscious in the ICU) that my body had processed so much of the medication I used to overdose on (full bottles of aspirin, depakote, and tegretol… to treat depression) that they could not pump it all out and they didn’t know how I was still alive at all, but that it would be imperative for me to have my liver and kidney levels checks on a regular basis beginning after 5 years from that point… I’m now 33 and it has been 16 years, 9 years since my last suicide attempt. COULD this be the cause? If so, I’m cool with this, I just want to know.

    1. get hcv test. make sure you don’t have any type of hepatitis. if not get a comp. metabolic panel. You may have some perm damage to your liver(scarring) from what you said, or it could be nothing. Only way to know is blood tests. Doctors only can speculate without blood work.

    1. alt is slightly elevated. As I mentioned before, it could be anything. process of elimination. First HCV test. if negative, a comp. metabolic panel. if all good, then try cleaning up diet, quitting drinking, and upping water intake. moniter blood after a few months of this.

    1. elevated enzymes. get hcv test first, along with basic metabolic panel. it’s worth it just for piece of mind. if these tests are within range, look into diet,drinking, medications. it’s seems some people just have elevated enzymes wirh no real problems, but I say better safe than sorry, and blood work is better than speculation

  15. My recent metabolic panel shows my ast is 80 and als is 202. I hardly drink and when I do, I drink 1 serving. No drugs either. Cholesterol and tri is optimal but i am getting these high charts. Is this bad? I have read about the ratio part but don’t quite understand it.

My son is 22, does not drink alcohol but his father passed away from liver cancer (alcohol related) and his grandfather and great grandmother on his dad’s side both died from liver disease (not alcohol related). On 10/24/17 his AST was 27, ALT was 63. On 12/27/17 had blood work done again and AST was 27, ALT was 71. His diet is not the best, not horrible but could be healthier. Back in September he took Tylenol for a couple of weeks for headaches, and he just finished a round of antibiotics a couple of weeks ago. Everything else was normal. I am a bit concerned about the ALT because of family history- should I be?

I’m assuming this means ALT level, but I know VERY little about the liver… I know she’s an alcoholic and drinks all day every day and has for 10+ years. Could this be a different level?     My mother was diagnosed with ALS in May 2014. Her doctor put her on riluzole, letting her know there was no cure but the medication might provide her a few more months of delayed symptoms.     My ALT is 145 and AST is 94.
    I am a healthy 26 year old, they ordered an ultrasound but it?s not for two weeks and I?m panicking.
    I?ve been getting nauseous after eating, small cramps on my right side, and general fatigue. I also lost 11 pounds recently and recently began Wellbutrin.

    Anyone have any advice? Should I be worried or does this sound pre-cautionary.

  27. I have fibromyalgia, and it was painful on some days. I took methocarbamol to relieve the pain. Sometimes it helps and other days it doesn’t. I had it for almost 16 years, there was no cure. So I had to live with it the best that I could. The symptoms I had were Muscle Pains, mood swings, and difficulty falling asleep.There has been little if any progress in finding a cure or reliable treatment.   28. My ALT levels are 100 and my serum ferritin is 953. I do not drink alchol and I am not fat but I do have short bowel syndrome (1.2 m left). Anything I can do ?

  29. My AST came back as 117, and my ALT was 110. When I went to the doctor, I was complaining of fatigue, low grade fever, body aches and joint pain and swelling. I haven’t noticed any itching or changes in my stool or urine. I have fibromyalgia, but don’t take anything for it usually except Ibuprofen, naproxen and cyclobenzaprine 10 mg a day. I work out regularly and have a full time job. I’m going in Friday the 9th of Dec for more tests. Could this be mono?
